I've not seen this form before, perhaps because I rarely look at monstrous plants.
I don't know where it came from, but it seems that there some difference between this and painteri monstrosus if you look at the Plapp website. There this plant looks much more flattened, and with different flower buds. But on Cactus-Art, the plants look the same.
